Initial Comments:
Based on the findings of an offsite follow-up survey completed March 21, 2022, OPSTA @ Home was found to have corrected the deficiencies cited under the requirements of 28 Pa. Code, Part IV, Health facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of an unannounced on-site State Re-Licensure survey completed on October 20, 2021.
